What is the difference between accredited and non-accredited Caribbean med schools?

been researching caribbean med schools and keep seeing this accredited vs non accredited thing but the more i read the more confused i get. some schools say nationally accredited, some mention CAAM-HP, some mention ACCM. have no idea what any of it means in practice. what actually happens if you graduate from a non accredited school?

for a caribbean MD graduate wanting to practice in the US two things matter. first is CAAM-HP which is the caribbean accreditation authority for education in medicine. second is ACCM which is the accreditation commission on colleges of medicine. your school needs one of these and needs to be recognized by NCFMEA and listed as ECFMG eligible in the world directory of medical schools

non accredited means none of the above. You likely cant get US clinical rotation slots, cant match into US residency, cant get federal loans. completely different situation

to add to what LeahCole (Student) said. the Big 4 are all CAAM-HP accredited and NCFMEA recognized: SGU, Ross, AUC, Saba. AUA and TAU are also accredited

the danger zone is the long tail of smaller schools that market aggressively to desperate premeds. they use words like approved, licensed, nationally recognized. none of which means anything for US practice. always look up the school yourself on the ACCM or CAAM list. if its not there, leave

Go to accredmed.org for ACCM or caam-hp.org for CAAM. Look for schools listed as Assessed Programs, that actually matters.

want to share a real story because abstract explanations dont fully land

a close friend enrolled at a small caribbean school that claimed to be fully accredited on their website. it was licensed by the local government which is not the same as ACCM or CAAM-HP. she completed 2 years of basic sciences, took out private loans, then discovered she couldnt register for usmle step 1 because the school wasnt ECFMG listed. she lost 2 years and roughly 60k dollars

please verify independently that the school is ECFMG listed before you pay a single dollar in deposits

few things even experienced students miss on accreditation

LCME accreditation is for US and canadian MD programs. caribbean schools will never have LCME. thats not the right bar

ACCM or CAAM-HP is the caribbean equivalent. this is the gold standard for caribbean schools

SGU is accredited by the Grenada medical and dental council which is WFME recognized. so slightly different path but still legitimate

accredited schools have established affiliations with US teaching hospitals for 3rd and 4th year. non accredited schools often cant place students in real affiliated programs. residency directors look at where and how you did your clinical training. a rotation at a recognized teaching hospital matters way more than a diploma from a school nobody knows

I want to address something that comes up a lot students who think they can start at a non-accredited school and transfer to an accredited one later to clean up their record. In practice this is extremely difficult. Accredited Caribbean schools rarely accept transfer credits from non-accredited programs, why would they risk their own reputation you’d likely have to restart from semester 1. the only real path is to get into an accredited school from day one. If you don’t meet the current admission requirements, do a post-bacc, retake the MCAT, build your GPA. it takes longer but it’s infinitely better than the alternative.

what separates people who match from those who dont is not just accreditation. its step scores, strong clinical evals, good letters, research if you can get it. accreditation is the floor not the ceiling. get into an accredited school then do the work

any school that passes both is worth seriously considering. any school that fails even one, walk away

angle nobody touched on. if you cant get into an MD or accredited caribbean school right now, DO programs are genuinely underrated. COCA accredited DO schools in the US are fully recognized for residency matching and merged with MD programs in the 2020 unified match. MCAT and GPA requirements are lower than US MD but not as low as some people assume. outcome potential for primary care and many specialties is essentially equivalent. worth having in your decision tree instead of going to a non accredited school out of desperation

adding a licensing angle. even if a non accredited school graduate somehow passed USMLEs, which is generally not possible without ECFMG certification, individual state medical boards in new york, california and florida all require graduation from a school listed in the world directory as ECFMG eligible. there is no workaround. you cannot be licensed to practice medicine in the US as a physician from a non ECFMG eligible foreign school. it is a hard wall

As a student, my biggest takeaway is this: don’t just trust words like government licensed or nationally approved. If your goal is to practise in the US, what really matters is CAAMHP or ACCM accreditation, NCFMEA recognition, and ECFMG/WDOMS eligibility.

If a school is not ECFMG-eligible, you may not be able to sit for USMLE, apply for US residency, or access US federal loans. And transferring out later can be very difficult, so it’s better to verify everything before enrolling.

Best places to check are WDOMS, ACCM, and the CAAM-HP list. accreditation is only the starting point, but choosing the wrong school can close doors before you even begin.

Late to this thread but wanted to add something. I work as an IMG residency coach so I see this stuff up close. Marcus's summary is solid but there's one thing worth spelling out more. There is actually a pretty big difference even within accredited schools. SGU and Ross have been placing students into residency for decades, they have relationships with ACGME programs and whole offices dedicated to this. A newer school might have gotten its accreditation boxes ticked but placed maybe a dozen graduates total. When you talk to admissions, ask them specifically for match rate data broken down by specialty over the last five years, not just the overall number they put on the website. If they give you a runaround on that question, that tells you something too.

Just finishing my first week at AUC and reading this whole thread made me feel so much better about my decision. One thing I hadn't thought to research on my own was the accreditation piece. My aunt is a nurse in the US and before I paid my deposit she basically sat me down and said you need to make sure this school is ECFMG eligible before you send them anything. I honestly didn't know what that meant at the time. She walked me through it. If you are first gen or your family doesn't have a medical background, please try to find someone already working in US healthcare before you sign anything. They will know which questions to ask in a way that no admissions brochure will tell you.

Jumping off what Charlotte14 said. I was actually a transfer student and the reason I had to transfer is exactly what this thread is warning about. The school I started at kept telling me they had provisional accreditation and that full accreditation was coming. What I found out after one semester is that provisional just meant they were working toward it with no guarantee they would actually get there. I lost tuition money and time but I caught it early enough to get out. The thing to watch for is when a school uses accreditation language but you can't find them on the ACCM or CAAM-HP list yourself. Don't take their word for it. Look it up independently. And if you see the words provisional accreditation, ask specifically who it is provisional with and what happens to students if they don't make it to full status.

Just want to push back a little on an assumption I see in a lot of these conversations, which is that bigger school automatically means better accreditation standing. Saba is tiny. Like genuinely tiny. But the accreditation history there is solid and our USMLE pass rates back it up. I have seen people pass over Saba because they assume a school nobody has heard of must be sketchier than a school with a bigger marketing budget. That's not how accreditation works. Do the research on each school individually. Size and advertising spend are not the same thing as accreditation quality.

Quick question for the thread. Does accreditation status ever change after a school already has it? Like can a school actually lose accreditation while students are already enrolled there, and if that happens what becomes of those students?

CalebHughes (Alumni) yes, it has happened and it is bad when it does. There have been real cases where Caribbean schools lost ECFMG eligibility while students were mid-program. What that means in practice is that students suddenly can't register for the USMLE, federal loan disbursements can stop, and the hospital affiliations for clinical rotations can fall apart. The accrediting bodies will usually give a school some runway to wind down and sometimes help students find transfer options at other programs but there is no guarantee of a smooth landing. It is one of the strongest reasons to go with a school that has been accredited for a long time rather than one that just got it last year.
